Current Opportunities
Dementia Support Navigator – Registered Social Worker
The Alzheimer Society of Calgary (Alzheimer Calgary) is a knowledgeable and credible voice for people with Alzheimer’s disease and other dementias and their care partners in Calgary and surrounding areas.
We are a trusted ally, striving to increase the capacity of people with dementia, their families and formal and informal care partners by connecting them with exceptional quality, evidenced-based knowledge and services.
Reporting to the Director, Learning and Support Services, the Dementia Support Navigator will provide practical and emotional support for persons impacted by dementia. Responsibilities include system and resource navigation, suggesting communication strategies, and providing emotional support. There may also be the opportunity for family and public education and group facilitation.
Key Responsibilities:
Dementia Support Navigation:
- Provide support for caregivers and persons living with dementia over the phone, via email, and through virtual and in-person meetings.
- Educate on Alzheimer’s disease and other related dementias.
- Provide care and communication strategies to maintain the quality of life for both caregivers and persons living with dementia.
- Assist in navigating community resources and services.
- Offer strengths-based and person-centred emotional support.
- Make appropriate referrals and linkages to meet clients' needs.
- Co-facilitate caregiver support groups as required.
- Facilitate Caregiver Strategies workshops as required.
- Provide administrative support for caregiver support groups as needed.
- Conduct site visits to other health, mental health, and community providers as necessary.
- Proactively gather information on the latest resources available in the Calgary area.
Technical Responsibilities:
- Monitor the support services inbox and distribute emails appropriately among the team.
- Complete progress notes in a timely manner utilizing Apricot 360.
- Maintain client confidentiality and adhere to all IT safety policies.
General Responsibilities:
- Participate in team meetings, informal team consults, and formal supervision.
- Contribute to creating and maintaining a positive and encouraging team environment.
- Support education sessions and community engagement activities as needed.
- Actively participate in an evaluation process to ensure programs are outcome-driven and sustainable, aligning with the Alzheimer Calgary's mission and vision.
- Participate in committees as identified by the Director of Learning and Support Services or guided by program strategy.
- Promote human rights, social justice, and respect for human diversity.
Professional Development:
- Attend Alzheimer Calgary professional development trainings.
- Complete an Accountability Agreement and strive to accomplish self-identified goals.
- Engage in formal and self-directed learning to stay informed of current trends and best practices in dementia care.
- Demonstrate a commitment to learning new skills, methods, and ideas by identifying relevant courses for self-development.
- Establish and maintain networks within the community to enhance current knowledge and awareness of programs and resources relevant to the role.
- Meet the continued competencies required for any regulatory body governing practice (e.g., ACSW).
Qualifications:
- A Bachelor of Social Work degree and professional registration.
- Demonstrated ability to provide trauma-informed, culturally safe, person-centred care.
- Self-directed learners with the ability to research information and resources as needed.
- Exceptional time management and organizational skills, including the ability to prioritize workload.
- Strong understanding of Alzheimer’s disease, dementia, and caregiving issues.
- Excellent written and verbal communication skills.
- Exceptional organizational, analytical, and interpersonal skills are required.
- Proficiency in Microsoft Office Suite and comfort navigating virtual platforms such as Zoom are required.
- Compassionate and empathetic approach to working with individuals affected by Alzheimer’s and their families.
