Dementia Support Navigator - Community Inclusion

Reporting to the Director of Programs, the Dementia Support Navigator – Community Inclusion supports individuals and families impacted by dementia through culturally safe navigation, emotional support, system coordination, and community connection.

This role also supports Alzheimer Calgary’s commitment to diversity, equity, inclusion, and culturally responsive service delivery by building relationships with equity-deserving communities, identifying barriers to access, and supporting the implementation of organizational DEI priorities.

The position combines direct client support with community development, partnership building, and internal collaboration to help ensure services are inclusive, accessible, and responsive to the diverse experiences of people living with dementia and care partners.

The role works closely with organizational leadership and external partners to strengthen dementia-inclusive communities and improve equitable access to supports and services.

Key Responsibilities

Dementia Support Navigation

  • Provide emotional support, education, and system navigation for people living with dementia and care partners through phone, virtual, and in-person support.
  • Support individuals and families in understanding dementia, available services, and care planning options.
  • Assist clients in navigating health, community, financial, housing, and social support systems.
  • Provide strengths-based, trauma-informed, culturally safe, and person-centred support.
  • Facilitate or co-facilitate support groups, workshops, and education sessions as required.
  • Maintain accurate documentation and progress notes within Apricot 360 software.
  • Make appropriate referrals and linkages to internal and external community resources.

Equity & Community Inclusion

  • Build and maintain relationships with ethnocultural, Indigenous, 2SLGBTQ+, rural, and other equity-deserving communities and organizations.
  • Support outreach efforts to increase awareness of dementia supports within underserved communities.
  • Identify barriers to accessing dementia services and recommend practical solutions to improve inclusion and accessibility.
  • Support adaptation of programs, resources, and approaches to better meet diverse community needs.
  • Participate in community events, collaborative initiatives, and sector partnerships that strengthen dementia-inclusive communities.
  • Help identify opportunities for partnership, co-design, and community-led initiatives.

Organizational DEI Support

  • Support implementation of organizational DEI priorities and roadmap initiatives in collaboration with leadership and external consultants.
  • Provide frontline insight and community-informed perspectives to help shape service improvements.
  • Contribute to internal discussions related to culturally responsive practices, inclusion, and accessibility.
  • Assist with identifying relevant community resources, partnerships, and learning opportunities for staff and programs.
  • Support evaluation and data collection related to equitable access and community impact.

Community Engagement & Education

  • Deliver dementia education and awareness sessions to community organizations, professionals, and community groups as required.
  • Support relationship-building with health, social service, and community organizations across the Calgary region.
  • Maintain awareness of emerging trends, resources, and best practices related to dementia care, inclusion, and community development.

General Responsibilities

Qualifications

  • Bachelor of Social Work degree required.
  • Registration with the Alberta College of Social Workers.
  • Experience providing trauma-informed, culturally safe, person-centred support.
  • Strong understanding of dementia, caregiving, and community-based supports.
  • 3 years experience working with diverse communities and/or community development approaches.
  • Demonstrated ability to build partnerships and relationships across sectors and communities.
  • Strong communication, facilitation, and interpersonal skills.
  • Experience with systems navigation, advocacy, and community resource coordination.
  • Strong organizational and documentation skills.
  • Experience with Apricot or similar databases considered an asset.

Community Engagement Facilitator

Reporting to the Director of Programs, the Community Engagement Facilitator supports Alzheimer Calgary’s education, outreach, and community engagement initiatives through the delivery of accessible dementia education, community relationship-building, and inclusive engagement opportunities.

This role supports caregivers, professionals, volunteers, and community members through education, event coordination, outreach, and partnership development. The position also contributes to Alzheimer Calgary’s commitment to diversity, equity, inclusion, accessibility, and culturally responsive service delivery by helping reduce barriers to participation and strengthening connections with diverse communities.

The role combines community engagement, education delivery, event coordination, and collaborative partnership development to support dementia-inclusive communities and increase awareness and access to Alzheimer Calgary’s programs and services.

Key Responsibilities

Education & Community Engagement

  • Plan and deliver dementia education sessions, workshops, presentations, and learning opportunities for caregivers, professionals, community organizations, and the public.
  • Support professional education initiatives, including Alzheimer Calgary's 'ABC Pro' and other community learning opportunities.
  • Assist with the development and adaptation of educational resources and materials that are accessible, inclusive, and culturally responsive.
  • Support outreach and relationship-building efforts to increase awareness and participation in Alzheimer Calgary’s educational offerings.
  • Promote understanding of the diverse experiences of people living with dementia and care partners across communities.

Event Coordination & Community Gatherings

  • Support planning, coordination, promotion, and execution of community events, educational gatherings, support group events, and engagement initiatives, including 'Talking About Dementia' and other Alzheimer Calgary events.
  • Collaborate with staff, volunteers, sponsors, vendors, and community partners to support successful event delivery and participant experience.
  • Assist with event logistics, communications, registrations, resource preparation, and evaluation activities.
  • Support welcoming, accessible, and inclusive gatherings that foster connection, peer support, and community engagement.

Community Partnerships & Inclusion

  • Build and maintain relationships with community organizations, professionals, and diverse communities to strengthen access to dementia education and supports.
  • Support Alzheimer Calgary’s organizational commitment to diversity, equity, inclusion, accessibility, and belonging through outreach and engagement efforts.
  • Identify opportunities for collaboration, outreach, and community capacity building.
  • Represent Alzheimer Calgary at community events, outreach activities, and collaborative initiatives as required.

Evaluation & Organizational Support

  • Gather participant and stakeholder feedback to support continuous improvement and responsive program development.
  • Support data collection, reporting, and evaluation activities related to education and community engagement initiatives.
  • Collaborate with team members to ensure programs remain relevant, sustainable, and aligned with organizational priorities.
  • Participate in team meetings, organizational initiatives, and professional development opportunities.

Qualifications

  • Diploma or degree in Human Services, Education, Community Development, or related field.
  • Minimum one year of experience in community engagement, education, event coordination, public speaking, or dementia-related services.
  • Experience working with diverse communities and inclusive engagement approaches considered an asset.
  • Knowledge of dementia and adult learning principles considered an asset.
  • Proficiency with Microsoft Office and virtual platforms including Zoom.

This position requires flexibility to support in-person and virtual engagement opportunities, including occasional evening and weekend events.
